TEMPORARY CARD REQUEST
For the duration of the COVID-19 pandemic, the Library will be offering temporary library cards to anyone in town who does not yet have a card. We are not currently checking out physical materials, but this will give cardholders access to our e-resources.
Use the form below and we will create your temporary card and will get back to you with the details. At this time, we are ONLY able to process requests that have a working email address to receive a temporary card.
NOTE: This card is only available to those that live in the town of Middlesex Borough, with a zip code of 08846. Must have a working email address to receive a response. All fields are REQUIRED.

CARD NUMBER/PIN REQUEST
Under normal circumstances, we do not give PIN numbers or Library card numbers via email or phone. However, due to these unprecedented times, we are temporarily providing card numbers and PINS remotely. Please indicate your need on the drop down menu below.
